The DAWG Nation Hockey Foundation is a nonprofit organization dedicated to promoting the spirit of fellowship via recreation hockey competition, supporting players and player's families in high standards of health, wellness, and fair play, and supporting league and team players in local community outreach.

NOMINATE SOMEONE FOR ASSISTANCE - Initial ONLINE Application - a follow-up more detailed application will be requested if the foundation is considering the Intial ONLINE Application submitted.

PURPOSE:
A assistance grant will be given when financially viable by the DAWG Nation Hockey Foundation to eligible recipients. The DAWG Nation Hockey Foundation will be responsible for disbursement of funds when applicable. Monies will be sent directly to the individual's family or medical organization as deemed appropriate by the Dawg Nation Hockey Foundation. If the recipient decides not to use the monies or declines assistance, the funds will be returned directly to the DAWG Nation Hockey Foundation.

ELIGIBILITY:
Assistance grants are available to any and all adult hockey players and/or their families who can show financial hardship due to health and/or medical related burden.

PROCEDURE:
Assistance Grant Applicants should submit the following information to
DAWG Nation Hockey Foundation

Must include the following:
" Completed and signed application
" Medical Condition
" Financial Hardship

AWARDS:
The grant and dollar amount will be voted on by the Dawg Nation Board of Directors
